Fuel Surcharge Information

Due to the continued rise in the cost of diesel fuel, we revised our fuel surcharge index, effective May 1, 2005. The surcharge will remain in effect as long as the U.S. National Average Diesel Fuel Index remains above $1.25 per gallon.

The level of the fuel surcharge is adjusted on Wednesday based upon the U.S. National Average Diesel Fuel Index of the prior Tuesday. This index, published by the Energy Information Administration of the U.S. Department of Energy, is available by telephone at
202-586-6966 and on their web site.

The national average cost for diesel fuel for the week of May 13, 2013 is posted at
$3.866 per gallon.

If the cost of fuel exceeds the figures in the table, then for every $0.05 cents per gallon change,
an additional .5% in surcharge will be applied.
